Oaksprings Counseling (“we,” “us,” or “our”) is committed to protecting your privacy. This Privacy Policy explains how we collect, use, and safeguard information you provide when you visit our website at oakspringscounseling.com or contact us to inquire about or receive counseling services.
This policy applies to information collected through our website. It does not govern the handling of protected health information (PHI) collected in the course of providing clinical services – that is addressed in our separate Notice of Privacy Practices below.

5. HIPAA and Protected Health Information

As a licensed clinical social worker (LCSW) who bills insurance, Lucille (Lucy) Hamblin is a HIPAA-covered provider. Protected Health Information (PHI) – meaning health information that identifies you – is governed separately by federal HIPAA law and our Notice of Privacy Practices (see below), not by this website Privacy Policy.


6. Cookies and Tracking Technologies

Our Commitment to Your Privacy

Oaksprings Counseling is committed to protecting the privacy of your health information. As a HIPAA-covered provider, we are required to:
Maintain the privacy of your Protected Health Information (PHI)
Provide you with this Notice of our legal duties and privacy practices
Follow the terms of this Notice currently in effect
Notify you in the event of a breach of your unsecured PHI


What Is Protected Health Information (PHI)?

PHI is information about you – including demographic data – that relates to your past, present, or future physical or mental health or condition, the provision of health care to you, or payment for that care, and that identifies you or could reasonably be used to identify you.


How We May Use and Disclose Your Health Information

Treatment: We may use and disclose your PHI to provide, coordinate, or manage your mental health care.
Payment: We may use and disclose your PHI to obtain payment for services, including submitting claims to your insurance carrier (Aetna, Cigna, UMR, UnitedHealthcare, or Meritain Health) through the Tava platform.
Healthcare Operations: We may use and disclose your PHI for internal business operations, including quality assessment, training, and licensing compliance.
Required by Law: We will disclose your PHI when required by federal, state, or local law, including mandatory reporting obligations under Arizona law.
Public Health and Safety: We may disclose PHI when necessary to prevent a serious and imminent threat to health or safety.
Child Abuse or Neglect: We are required by Arizona law to report known or suspected child abuse or neglect.

Right to Confidential Communications: Request we communicate with you in a specific way or location.
Right to a Paper Copy: Receive a paper copy of this Notice at any time.
Right to Breach Notification: Be notified if your unsecured PHI is breached.

Telehealth Informed Consent

Effective Date: May 4, 2026

In accordance with Arizona law (A.R.S. § 36-3602), your informed consent is required before telehealth services may be provided.


What Is Telehealth?

Telehealth is the delivery of mental health services using secure video technology rather than in-person contact. At Oaksprings Counseling, all sessions are conducted virtually via a HIPAA-compliant video platform.


Benefits of Telehealth

Convenient access to therapy from home or another private location
Eliminates transportation barriers
Consistent care regardless of geographic distance within Arizona


Potential Risks and Limitations

Technology failures: Internet or platform disruptions may interrupt sessions
Confidentiality risks: You are responsible for ensuring your environment is private
Clinical limitations: Some conditions may be better suited to in-person care
Emergency response: Remote coordination of emergency assistance may be more difficult


Your Responsibilities

Use a private location where you cannot be overheard
Use a reliable internet connection
Avoid driving or public spaces during sessions
Do not record sessions without prior written consent


Emergency Procedures

Oaksprings Counseling is not a crisis service. In a mental health emergency:
Call 988 (Suicide & Crisis Lifeline)
Call 911 or go to your nearest emergency room
Text HOME to 741741 (Crisis Text Line)

Please provide your current address at the start of each session so your therapist can contact emergency services on your behalf if needed.


Licensing

Lucille (Lucy) Hamblin is a Licensed Clinical Social Worker (LCSW) licensed in Arizona. Services are provided to clients located in Arizona at the time of the session. Please notify your therapist if you will be outside of Arizona during a scheduled session, as that session will need to be rescheduled.


Confidentiality

The same confidentiality protections that apply to in-person therapy apply to telehealth. Sessions will never be recorded by Oaksprings Counseling without your explicit written consent. Please refer to our Notice of Privacy Practices above for full details.


Your Right to Withdraw Consent

You may withdraw consent for telehealth services at any time, for any reason, without affecting your right to receive other appropriate services.


Your Agreement

By scheduling and attending a telehealth session with Oaksprings Counseling, you confirm that you have read and understood this consent, have had the opportunity to ask questions, consent to telehealth services, and are located in Arizona at the time of each session.

1. Agreement to Terms

By accessing oakspringscounseling.com or scheduling counseling services, you agree to these Terms and Conditions.


2. Not a Crisis Service

Oaksprings Counseling is not a crisis service and is not monitored 24/7.
In an emergency: call 988, call 911, or text HOME to 741741.
Do not use website contact forms or email to communicate urgent or emergency needs.


3. Professional Relationship

A therapist-client relationship is established only after a formal intake process with clients located in Arizona. Nothing on this website constitutes therapy, clinical advice, or a professional relationship.


4. Scope of Services

Oaksprings Counseling provides individual therapy for adults dealing with trauma, depression, PTSD/C-PTSD, relationship challenges, life transitions, and related concerns via telehealth.
We do not provide:
Crisis intervention or 24/7 support
Medication management or psychiatric evaluations
In-person sessions
Court-ordered evaluations or forensic services (unless separately arranged in writing)
Services to minors without appropriate consent and clinical suitability review


5. Appointments and Cancellations

Cancellations: Please provide at least 24 hours’ notice to cancel or reschedule. Late cancellations or no-shows may be subject to a $100 fee as outlined in your individual service agreement.
Tardiness: Sessions may be forfeited if you are more than 15 minutes late without prior notice.


6. Fees and Payment

Session fees are discussed and agreed upon prior to your first appointment. Payment is due at the time of service.

Insurance: Oaksprings Counseling accepts Aetna, Cigna, UMR, UnitedHealthcare, and Meritain Health through Tava. You are responsible for verifying your benefits and for any amounts not covered by your insurer.

Good Faith Estimates (GFE): Uninsured and self-pay clients have the right to receive a Good Faith Estimate of expected costs before scheduled services, in accordance with the No Surprises Act. A brief overview is provided below, with a detailed GFE provided before services begin:

The overall cost of therapy depends on how often you have appointments, and how long you and your therapist determine services are required. Sessions cost $140 per 53-60 minute session, including initial intake (90837, 90791); $122 per 38-52 minute session. (90834); $87 per 16-37 minute session. (90832)
$100 for late cancellation, no-show fees (99002, 99232).


7. Confidentiality

All sessions and client information are confidential per HIPAA and Arizona state law. Exceptions exist for imminent risk of harm, suspected abuse or neglect, and court orders. See our Notice of Privacy Practices above.


8. Website Disclaimer

Information on this website is for general informational purposes only and does not constitute medical or mental health advice, diagnosis, or treatment.


9. Limitation of Liability

To the fullest extent permitted by law, Oaksprings Counseling and Lucille (Lucy) Hamblin, LCSW shall not be liable for indirect, incidental, or consequential damages arising from use of this website or reliance on its content.


10. Governing Law

These Terms are governed by the laws of the State of Arizona.


11. Changes to These Terms

We reserve the right to update these Terms at any time. The “Last Updated” date at the top reflects the most recent revision.
